Re: 45 - 49 year old Health Check
We recognise that it is better to prevent any disease than to treat it and have introduced a routine health check for everyone aged between 45 and 49 who may be at risk of developing a health complaint, like diabetes or heart problems later in life.
This health check will give us the opportunity to look at your lifestyle, medical and family history to identify potential risk factors and put a plan in place to manage these, lessening the likelihood of you going on to develop a chronic disease.
The health check will involve:
· Updating your medical history and looking at your health and lifestyle issues
· Doing tests (such as blood pressure, checking cholesterol and skin checks), if required
· Follow up of any problems identified
· Advice and information; i.e. how to make lifestyle changes to improve your overall health and wellbeing.
Risk factors include :
| High blood pressure High cholesterol |
| Lack of physical activity Poor diet |
| Extra weight gain Smoking Alcohol use or Gambling |
| Family history of cancer, diabetes, heart disease |
Our records show that you are within the age range for the health check. If you have a risk factor like a family history of the above named conditions and have not already had this health check, please phone the practice on 8359 6266 for a "45- 49 year old health check" appointment with our nurses and your regular doctor . The health check is bulk billed as it is covered by Medicare and thereby, no cost for this service.
